ADMISSIONS & DISCHARGE

Surecare Children’s Home offers a systemic care programme for young people aged 12-16 years regardless of sex, ethnic, racial and cultural background, who are looked after by Local Authorities. These young people may well exhibit emotional and behavioral difficulties (EBD), and may have experienced abuse, trauma and other forms of emotional and psychological problems.

Surecare Children’s Home takes both planned and emergency admissions. Wherever possible, admission should be planned in advance. However, young people referred for emergency admission will be adequately prepared for and be made to feel comfortable. Under no circumstances should a young person be admitted without a risk assessment carried out on them.

The Operations Director will deal with all referrals in consultation with the home manager and the office manager. The final decision to admit a young person must rest with the Director. The manager is required to exercise discretion in emergency referral cases. All referrals must be logged and all relevant details entered into the relevant records.

Information exchange

Any phone calls about referrals will be passed on to management as soon as they come though. The member of SureCare staff will take down relevant contact details from the authority phoning ensuring that name and phone number of the person calling is noted.

Planned Admissions

A social worker will make an initial enquiry which must be referred to the Director. If The Director is not available, a note should be made of the social worker’s name and telephone number for a return call to be made by the manager when available.

After a preliminary telephone discussion with the social worker, the home manager may invite the social worker, young person and their parents/guardian to an introductory visit. During the referral process and prior to the introductory visit, SureCare will require background information concerning the child including supporting documentation from previous placements such as Educational, Psychological or incident reports.

These reports will be read prior to the visit and the contents and the composition of the existing group will be borne in mind when assessing whether a placement at a SureCare home is appropriate.

During the visit, the young person and visiting adults will be shown around the home and made aware of guidelines. It is best if the young person agrees to being placed at the home soon after the visit. This helps to ensure that the young person has not felt intimidated and coerced into agreeing to the placement.

When the placement is confirmed, a date must be agreed with the social worker for a review meeting to be held. This meeting, with social worker, parents/guardian and key worker, is to discuss the placement and develop a care plan for the youngster. The meeting must be held no later than 28 days from admission. The care plan will be raised immediately on agreement of admission. At the meeting, a date not more than three months after admission is to be agreed for an interim review. Thereafter, subsequent reviews must take place at intervals of not more than six months.

Referrals to the team

The referral will be discussed with the staff on duty at shift changing meetings and during the next staff meeting. All staff must also read the referral information and any risk assessment which has been carried out.

Pre Placement visit

The home manager or deputy together with the allocated key worker must be available at the time of the initial introduction of the young person.

For young people and those with parental responsibility, change, including introductory visits, involves much anxiety. The young person may have experienced numerous placements and moves and these factors must be a primary consideration in handling the introduction to a new environment.

During introductory visits time must be allowed for

  • the young person and/or family members to meet and speak to staff and seek support and clarification of procedures

  • the home manager and key workers to establish a rapport with the young person

  • social workers to inspect the home and review its policies and procedures

  • incorporating, where possible, within the visit, an activity period, such as a meal, which allows the young person to experience a normal daily occurrence and the atmosphere

  • establishing financial details

  • scheduling the completion of the practical admission process (transferring process file/reports etc.)

Consultation with young people

Management will consult the young people living in the home about the new admission. It should not be inferred that their consent is required before a new person can move in. It is rather to prepare them for the arrival of the new resident. This should take place in a young peoples’ meeting and with a member of the management team present.

Emergency admission

Such an admission is usually made for a young person in the midst of a crisis. This is, in the main, for medium term or respite care with a view to rehabilitation to their own home or back to the previous placement.

Enquiries about placements of this type must be made to the Director who is responsible for any decision regarding admission.

A planning or placement meeting will be held within 72 hours of placement and an outline care plan formulated for the young person. Prior to the meeting, the social worker will be requested to provide all necessary background information and relevant recent reports.

At the meeting, an interim review meeting date not more than three months after admission will be agreed. Thereafter, reviews must take place at intervals of not more than six months.

Admission guide lines for all staff

  • Ensure that you read all current information on the young person moving in.
  • A key worker pack is to be started and explained to the young person. A key working contract needs to be signed by both the key worker and the young person.
  • An inventory of clothing and other items being brought in to the home is to be completed with the young person.
  • Observe behaviours and log them in their daily case notes making sure that, in the handover, any information is passed over no matter how insignificant it might appear.

Risk assessment

For any admission of a young person, a risk assessment has to be conducted.
The social worker must be informed, both by telephone and in writing, of serious incidents, e.g. absconding, drug, alcohol and/or solvent abuse, violence, destructive behaviour, problematic sexual behaviour, known sexual contact with another resident and incidents involving extreme verbal or physical abuse. This may be done through the incident reporting procedure.

Admission process

  1. The social worker or escort is to be present with the young person at the placement.
  2. A service agreement is to be provided to the Director. Basic information about the young person and LAC forms are to be provided to the responsible person/key worker. The registered manager is to record details of referral in the Admissions & Discharge Register.
  3. The young person is to start a key working pack assisted by the key worker. Any significant health concerns can be raised and an interpreter can be used if required.
  4. All details of the young person are to be recorded and a case records file made up.
  5. The young person must be made to feel welcome, be offered food and introduced to the other residents.
  6. The young person is to be given a welcome pack which should be read through with the key worker.

Working together

  1. The young person will be allocated to a key worker by the manager or senior deputy on duty.
  2. The key worker and staff on duty must ensure the young person is made aware of fire precautions and health & safety procedures.
  3. A planning meeting with the allocated social worker is to be convened within 3 days of admission of the young person.
  4. Relevant LAC forms and care plans are to be provided by the allocated social worker at the planning meeting if they were not available at admission.
  • Essential Information Records Part 1 is required immediately or within 3 working days following the emergency placement.
  • Essential Information Record providing detailed background information is to be completed within 14 days of the emergency admission.
  • Care Plan setting out objectives for the care of the Young Person and strategy for achieving them should be completed with 72 hours of placement.
  • Placement Plan Part 1 (Placement Agreement) must be completed detailing the immediate agreement to the accommodation and medical treatment (by those holding parental responsibility).
  • Placement Plan Part 2 (day-to-day arrangements) is to be completed jointly by the allocated Social Worker and Key Worker within 14 days of the placement clarifying arrangements.
  • The Review Form provides a framework for assessing whether the agreed day-to-day arrangements continue to meet the Young Person’s need – Statutory obligations to be met, etc. Part of the Review process is that the Young People and Carers are asked to make their views known by filling out the relevant consultation paper.
  • Confirmation of Local Authority support letter dated signed and stamped.
  • The key worker is to register the young person with health services including GP, dentist and optician.
  • The key worker is to register the young person with the Local Education Authority.
    The Referring Agency to be informed in writing if there is no vacancy and/or of any refusal of a school place received.
  • The key worker is to facilitate any other independent agency such as solicitors.
  • The key worker is to provide 3 monthly regular Progress Reports for the Young Person for Statutory Reviews.
  • The key worker is to attend a first statutory review four weeks after the young persons admittance, then three months later and thereafter at intervals of six months until the young person is no longer looked after.
  • Any disruption meeting will be convened within 24 hours at the placement address. A placement may be cancelled if the Young Person places others or self at Risk.

Discharging a young person

The departure of a young person will be as follows:

  1. Having negotiated a leaving date with the young person and all concerned (social worker and family), the young person will be given the opportunity to talk through their feeling of separation and loss with adults of their choice thus enabling them to make the transition as smoothly as possible. The young person will be made aware of the date of their leaving well in advance which will enable them to prepare for this with their chosen adults and their key worker.

  2. The child’s key worker should ensure that the young person is properly clothed and prepared for discharge

  3. The child/young person should be provided with their own suitcase and bags to transport their belongings. Young people should not be moved with their belongings in refuse bags. The symbolic effect of this can be very damaging.

  4. Any young people leaving a SureCare home should be able to do so with dignity and ensure that their experience in our care has been a positive one. The staff will mark the occasion by some significant event (eg, a leaving tea, supper or organised group outing with the giving and receiving of gifts).

  5. The staff team must aim to enable the transfer of positive, healthy attachments when working with young people at Surecare homes.

  6. The details of discharge must be recorded in the Admission/Discharge Book. When required, the key worker must prepare a detailed report for use at the young person’s next placement. The report must be supervised by the manger or a senior team member and a copy sent to the social worker.
